The following documents need to be presented to the Division Commissioner with this checklist printed out prior to a team receiving their uniforms:
AYSO National Volunteer Registration (InLeague)
(volunteer MUST re-register EVERY year)

∏ All Assistant Coaches
∏ Team Manager
∏ All Referees
∏ All Assistant Referees
∏ All Practice Chaperones (n/a for u5 and U6)
∏ TM has visually checked a photo ID of all volunteers above
Safe-Haven Certification (must be taken once, 2011 or later, from eTrainU)
(Verify prior certification by locating volunteers on InLeague and checking their certifications)

∏ All Referees
∏ All Assistant Referees
∏ All Practice Chaperones (n/a for u5 and U6)
Player Registration Forms (from inLeague–the registration system)
∏ 3 signed copies of EACH player’s Registration/Medical Release Form. (Team Manager needs to show the 2 copies to the DC, and DC will keep 1 full set per team.
Team Manager has access to print the form for each player through the registration system (inLeague).
Signed Kid Zone Pledge
∏ 1 signed copy of the Kid Zone Pledge for player on the team (Team Manager can print a form for each player’s parent(s) from the website)
Team Volunteer Roster (submitted via inLeague)
